WHEN YOU LEAVE 

EXTENDED CARE PLANNING
Any final arrangements concerning possible additional healthcare needs following discharge should be made at this time. For more information, refer to the Planning Ahead section of this book.

DISCHARGE PROCESS
On your day of discharge, be sure you have made arrangements to be picked up by 11:00 am. You will receive a discharge instruction sheet from the nursing staff. You or a member of your family will need to go to the Office of the Cashier, on the main floor of the Hospital, where a discharge slip will be given. This document should be taken back to your nursing station. You may then go home.
