A long-term acute care hospital is an extended acute hospital; the hospital gives acute care patient who condition is predicted to have a need for longer than usual acute care services.

The Continuing Care Hospital is accredited by the Joint Commission on Accreditation of Health Care Organizations (JCAHO) and is licensed by the Pennsylvania Department of Health. In addition NPHS shows:

What is the facility like?

Current Bed capacity: 9 intensive care, 44 Medical/Surgical Beds

NPHS’ advantage is a health information network that electronically links doctors, nursing facilities, clinics, and other services within the commitment to both St. Joseph’s and Girard Medical Center.  This linkage of healthcare resources enables the integration of clinical information and allows for better coordination of patient care.  The NPHS advantage health information network serves as a repository for patient data (inpatient/outpatient, ER, ambulatory.)  Healthcare providers can achieve a higher level of patient care because patient information is accessible in their own setting.

What is entailed in making a referral?

A referral is available 24 hours a day.  The majority of our patients are referred directly from intensive care units in other area hospitals.  Primary care physicians may refer patients into a program where they can visit the center and participate in all treatment and management decisions.  For referrals, please call (215) 787-2395.

What role does the attending physician from the hospital play?

To achieve that continuity, CCH invites referring physicians to apply for privileges at the hospital and to join the CCH staff in managing the patient’s medical care throughout the length of their stay.  Although upon the patient’s discharge, all patients are referred back to their primary physician for post-care.
